These emails are critical to complete the onboarding process.

In your welcome email, you will receive a link to the electronic Health History Questionnaire which will need to be completed prior to your pre-employment onboarding appointment. Once completed the Employee Health team will review your information and will reach out to schedule an appointment and collect any additional information needed.

If an in-person appointment is required the Employee Health team will reach out to schedule an appointment prior to your start date. This is separate from your appointment with HR and both must be completed prior to your start date.

For additional assistance on the Health History Questionnaire or to schedule an appointment please reach out to Employee Health at (713) 500-3267 or [email protected].
